What does New Leaf Primary Care offer its patients?New Leaf Primary Care provides the same medical services you expect from any other traditional brick-and-mortar office-based medical clinic. We provide the following services: comprehensive physical examinations, annual Medicare wellness visits, medication prescription and management, chronic disease evaluation and management, diagnostic testing (such as EKGs, x-rays, and ultrasounds), blood work and lab testing, immunizations, hospice, and palliative care, diabetic and vision screenings, wound care and more. We also provide complete care coordination, working with patients, their families, and their insurance companies to coordinate with specialists, home care services medical device and equipment deliveries, and when more involved testing is required.

What if I have to go to the hospital?We do not see patients in the hospital. If you are admitted to the hospital you will be seen by a hospitalist. We do work with the local area hospitals and will continue your health care before, during, and after your hospital stay.

Can I still see my regular doctor?New Leaf Primary Care recommends that you utilize one doctor as your primary care physician.

What if I need to contact my provider after hours?New Leaf Primary Care does not provide on-call / after-hours service. You may leave a message on the confidential voicemail which is screened often and if a non-urgent matter a call will be returned on the next business day. However, if you are having a medical emergency, please call 911 or go to your nearest emergency room.

Can I communicate via text messaging?Absolutely! Please know that safety, security, and the related cannot be guaranteed via text messaging. However, text messaging is another form of communication that is conducted.
